Public / Substitute Notice of Data Security Incident

On or about December 13, 2023, we were alerted to a data security incident. We promptly investigated and responded swiftly to this incident by enacting our existing response protocols, changing passwords, and blocking unauthorized access. Outside technical experts were engaged to determine the full scope of the incident and to further enhance the security of our systems. Law enforcement was also notified. Based on the investigation, it was determined that there was inappropriate access and/or acquisition of certain information on or about December 6, 2023.

We are in the process of notifying potentially affected individuals by first-class mail at their last known address – in an abundance of caution and as required by law. The potentially impacted files may have included data elements such as name, address, email address, date of service, prescriptions, and over-the-counter supplies – among potentially other elements used to process and seek reimbursement in connection with servic provided and/or in connection with employment. The files vary by individual. If a social security number or driver’s license number is at issue, we will provide complimentary credit monitoring and identity theft protection services as required by law.

Individuals potentially affected are encouraged to remain vigilant, carefully monitor financial account statements and credit reports, and report any discrepancies to law enforcement. Additional guidance can be found at: https://www.consumer.ftc.gov/features/feature-0014-identity-theft.
