The BioMatrix Points for Healthy Joints Program encourages patients to take an active role in the management of their bleeding disorder.
Every quarter participants have the opportunity to earn points for turning in a quarterly infusion log, completing a quarterly assessment with a pharmacist, attending an annual HTC visit, participating in BioMatrix educational programming, and returning signed delivery tickets.
Every point earned corresponds to a quarterly raffle entry with fantastic prizes!
How it works:
Active patients will automatically be enrolled.
Earn points from the first through the last day of the quarter by completing healthy behaviors.
We will track your points and provide individual raffle numbers ahead of the quarterly prize drawing.
For full transparency – the raffle will be conducted live via Zoom webinar - all participants are welcome to attend every third Thursday of the new quarter.
The raffle will be conducted using a numeric system. No patient names or information will be shared during the raffle.
Earning Points:
The program currently focuses on five healthy behaviors.
Every behavior carries a point value equal to the number of raffle entries received for completing that behavior each quarter.
Completion and submission of quarterly infusion logs (5 Points = 5 Raffle Entries)
Infusion logs may be submitted to Regional Care Coordinators or directly to the pharmacy
Paper or electronic logs are accepted
For MicroHealth users, assigning BioMatrix Specialty Pharmacy to your Care Team will result in points accumulating automatically (so long as logs are being actively maintained)
Successful quarterly health assessment completed with pharmacist (5 Points = 5 Raffle Entries)
Assessment must be completed by a pharmacist within 48 hours before or after receiving a medication shipment to earn points
Regional Care Coordinators can initiate a three-way call, patient can contact the pharmacy directly, or request that the pharmacist call during a certain time
Assessments typically take less than 5 minutes to complete
Participate in BioMatrix Educational Programming (5 Points = 5 Raffle Entries)
Attend a virtual or in-person BioMatrix educational program
Let your Regional Care Coordinator know you attended
Successful return of delivery ticket
(1 Point = 1 Raffle Entry)
Delivery tickets must be signed and returned within 30 days of receiving medication
Returned delivery tickets will be counted for the raffle in the quarter they are received
Points for delivery tickets will roll over to the following quarter's raffle if received within 30 days of shipment receipt
Successful completion of annual HTC visit (10 Points = 10 Raffle Entries)
During annual HTC visit, request that the HTC submit updated medical records or visit notes to the BioMatrix pharmacy to receive points
For patients new to BioMatrix, a verified visit within the past 6 months will also qualify for entry
Patients not followed by a federally funded HTC can provide visit notes from their regular hematologist/oncologist
